Release Forms
These forms are required to complete your annual recertification process.
You will need your email address and an email address for all persons 18 and over in your household to complete the online Release Forms. After you sign the form, you must go into your email and confirm your signature. Then, the form will be sent to any other adults in your household. They must also electronically sign the form for you to complete your annual recertification. If you do not have any other adults in your household, you will be able to click "Next" rather than enter additional email addresses.
